Clients may call OLA Records Division to inquire the total fee of the requested records or documents at (8)932-01-84.
MODE OF RELEASE AND PAYMENT
The requested records or documents shall be released not later than five (5) working days from date of request or acknowledgement, as the case may be. The said request shall be on a first-come-first-serve basis.
For clients who will personally claim the requested records or documents, proceed at the 4F Office for Legal Affairs - Records Division and present the written request or accomplished form and proof of payment (i.e. Official Receipt).
When the person to pick-up records or documents is a representative, the representative must bring with him/her authorization letter with his/her valid ID as well as the client's valid ID.
The payment fee for certified true copy of records or documents is Pl0.00/page. While, plain photocopy is P3.00/page.
